1. TO ENTER: submit your survey as instructed. You do not have to fill in any information on the questionnaire. To enter the sweepstakes without participating in the survey do the following: ”Scroll to the bottom of this page and click on the button “FORWARD Sweepstakes entry only.” Begins 05/15/08, and all entries must be received by 6/30/08. Limit one (1) online entry per e-mail address. Each online entry will be deemed to have been submitted by the record holder of the e-mail account from which the entry originated.

2. TO DETERMINE WINNERS: Winners will be selected in random drawings conducted on or about 7/20/08 from among all eligible entries received by 6/30/08. Random drawings will be handled under the supervision of MHK Creative Group, Inc, an independent agency whose decisions are final, and at whose offices (address listed in #7 below) winner selection is conducted. Winners need not be present to win.

3. PRIZES: 34 prizes will be awarded; There will be one Grand Prize,  two (2) second prizes, three (3) third prizes; four (4) prizes; twelve (12) fifth prizes and twelve (12) sixth prizes. Total estimated value of prizes: $2,500. Odds of winning depend on the number of questionnaires distributed and responses returned: estimated at 1 in 824. All prizes are guaranteed to be awarded. 4. ELIGIBILITY: Sweepstakes is open to residents of the 50 United States and D.C., ages 18 or older, to whom this offer is addressed. Employees and board members of the Forward Association, its affiliates, subsidiaries, advertising and production agencies, and their immediate families are not eligible. All federal, state, and local laws and regulations apply. Void where prohibited by law.

5. OTHER: Winners will be notified by mail and may be required to complete an Affidavit of Eligibility and Liability/Publicity Release which must be returned within 14 days of the date on the notification letter or alternate winners will be selected. Major prizes won by minors (as defined by their state of residence) will be awarded in the name of parent or legal guardian who must execute affidavit/release on behalf of minor. Any prize notification letter or prize returned as undeliverable will be awarded to an alternate winner. Limit of one prize per person per household. No substitution for prizes, except as may be necessary due to unavailability in which case a similar prize of equal or greater value will be awarded. Prize is not transferable. Taxes are the winner’s responsibility. Prize acceptance constitutes permission (except where prohibited) to use winner’s name and likeness for publicity purposes without additional compensation. If by reason of printing or other error more prizes are claimed in any prize category than the number set forth in these official rules, a random drawing will be conducted from all valid claims to award the advertised number of prizes. In no event will more than the advertised number of prizes be awarded. Sponsor and judges not responsible for late, lost, stolen, damaged, garbled, incomplete, misaddressed, or misdirected entries or communications; for errors, omissions, interruptions, deletions, defects, or delays in operations or transmission of information, in each case whether arising by way of technical or other failures or malfunctions of computer hardware, software, communications devices, or transmission lines or data corruption, theft, destruction, unauthorized access to or alteration of entry materials, loss or otherwise. Sponsor disclaims any liability for damage to any computer system resulting from participation in, or accessing or downloading information in connection with this Sweepstakes, and reserves the right, at its sole discretion, to modify, cancel, terminate or suspend this Sweepstakes should any virus, bug, technical failures, unauthorized human intervention or other causes beyond Sponsor’s control corrupt or affect the administration, security, fairness or proper conduct of the Sweepstakes. In the event of such cancellation, termination or suspension, a notice will be posted and a random drawing will be held from among all eligible entries received prior to such time. Sponsor reserves the right, at its sole discretion, to disqualify any entrant if his or her fraud or misconduct affects the integrity of the Sweepstakes.

6. CAUTION: ANY ATTEMPT BY AN ENTRANT TO DELIBERATELY DAMAGE ANY WEBSITE OR UNDERMINE THE LEGITIMATE OPERATION OF THIS SWEEPSTAKES IS A VIOLATION OF CRIMINAL AND CIVIL LAWS. SHOULD SUCH AN ATTEMPT BE MADE, SPONSOR RESERVES THE RIGHT TO SEEK DAMAGES FROM ANY SUCH ENTRANT TO THE FULLEST EXTENT PERMITTED BY LAW.
